With center locations in McPherson and Hutchinson, Kansas, Bluestem PACE (Program of All-Inclusive Care for the Elderly) serves those aged 55 and better living in Harvey, Marion, McPherson, Reno, Rice and Saline counties.

Bluestem PACE partners with seniors and their caregivers to manage complex medical issues while allowing them to continue living safely and independently in their homes for as long as possible. Participants receive an individualized comprehensive care plan that includes a variety of medical, personal and social services all from one source. 

Bluestem PACE participants can:

• Receive primary medical care
• Have transportation to specialist appointments
• Recieve in-home care
• Engage in social activities and recreation at Day Centers
• Get assistance with personal care needs
• And so much more!

Due to COVID-19, we have partnered with participants and their caregivers to delivery an adapted model of care that continues to deliver services. There is no better time to “live well at home” than during the COVID-19 pandemic with the partnership of the Bluestem PACE team.
